How to complete your scheduling worksheet

Completing the Google Request Form

Students will complete the Course Request Google Form when it becomes available during Advisory Feb. 8.  

You will absolutely need to have your scheduling worksheet on hand when you sit down to complete the Google form.  The course codes you will need to enter on the Google form are listed on the worksheet.  It may also be helpful for you to have your copy of the Program of Study on hand as well.

  1. After entering your first and last name, you will need to enter your School ID (your Timber teacher has it if you cannot remember it).
  2. The first four courses you will enter are core classes:  English, math, science and social studies. You are required to enter a course code for each one of these.  If you are taking one (or more) of these courses somewhere other than OHS, you will enter XXXX for the course code.
  3. If you want to take more than one math (or Science or social studies) course, you will enter one course code in the math (or science or social studies) section.  The additional course(s) will need to be added as an Elective further down in the Google form.
  4. Following the Core Classes, you will be required to enter a course code or an XXXX if you don’t want to take a world language, PE, health, or fine art course.  
  5. You have been provided many options to choose Electives; it is likely you won't need all of them.  The first two Elective options are required.  You will either enter a 4-digit course code or XXXX if you don’t have a choice.  The next five Elective fields are not required.  If you’ve already completed your requests, simply leave them blank.
  6. You are not able to request Study Halls.  They will be filled in for you, for example:  you request 6 courses, study hall will be filled in automatically for the open/available period.
  7. Review your course requests for accuracy.  Once you have done that, click the last option and submit your requests.
  8. FINAL task:  Open the email that lists your Course Request Google form results, and forward that email to your parent(s)/guardian(s)

Ohio’s College Credit Plus program allows students to earn college and high school credits at the same time by taking college courses from community colleges or universities either at the high school, online or on the college campus.
